1. Annexure A form ka printout nikale isako software se nahi bhare kabhi bhi. hath se bhare.
2. This form par applicant ka photo chipkaye/attech kare, yaha par vo photo add kare jo apane pan form apply karate time diya tha.
3. this form par jo photo lagate hai usako kisi Govt. Officer se verify karaye.
4. I hereby certify that I Know Shri.SMT.Kumari ke aage applicant ka name enter kare usake aage father name enter kare.
5. this form me jo table bana huaa hai usame Name option me applicant ka name enter kare jo usako pan card par chahiye.
6. Father Name, Date of birth me vo enter kare jo usake pan card par chahiye.
7. Residence Address me Applicant ka full address fill kare.
8. Office address ko fill karana Mandatory nahi hai isliye usako khali/Blank rakh sakate hai.
9. Agar aap Annexure A form se new pan card apply kar rahe hai tau Previous Name enter karane ka jarurat nahi hai, lekin jab aap pan card me correction karate hai tau apake pan card me jo old name hai usako enter kare.

Yaha govt officer ke details fill ki jayegi jisaka aap this form ko verify karate hai.

10. Office Address with Location me officer ka office wala address short me enter karana hoga.

11. Office Seal ki jagah par officer ko apana Seal lagana hoga.

12. Date, Place enter karna hoga.

13. Right Side me jo Signature ko option hai usake uper Officer ka apana signature karna hoga.

14. Full Name Me Officer ko apana jo bhi name hai vo enter karana hoga.

15. Designation me Officer ko apane Pad/Post Ka name enter karana hoga

16. Department/Organisation/Constituency me Officer ko apana Vibhag ka name enter karna hoga jis Vibhag ke uner me aate hai demo ke liye agar koi Headmaster hai tau usaka department Education Department hoga.

17. Identity Card NO. Me Officer ko Govt me prapt/Get Identity card me jo number diya hai ID Card ka Vo enter karana hoga.

18. Telephone/Mobile Number Dono me se koi ek number enter karana hai Officer ko apana.

Aise Aap this form ko fill kar sakate ho. Jab aap this annexure a form ko complete fill kar de tab apako this form ke sath Aap jis govt officer se verify karate hai usaka Identity card ka ek photocopy lena hoga usako annexure a form ke sath attech kar dena hai.

Identity Card me only Official Identity card hi lagega aap officer ka voter card/ aadhar card/ pan card/ passport etc. ID Card Used nahi kar sakate hai. Only Official Idenity card hi lagega. Without Identity card ke yah form work nahi karega.

Jab card annexure a form ke sath Identity card attech kar de tab apako pan form ke sath laga dena hai.
